The status (interim progress) reports should summarize and discuss the work performed during each period of performance, results obtained, and any technical or programmatic issues. A any technical or programmatic issues not addressed in the draft and Final Technical Report may be addressed in the letter or memorandum forwarding the associated report.
 
The final report must include a completed SF 298 -Report Documentation Page (as the first or second page) and scientific/technical progress. It may include proposed follow-on research and development, updated commercialization plans, and recommendations, as required.
 
Language used to report Phase I progress in a status report and/or Phase II proposal may be used verbatim in the final report, with changes to accommodate results after report and/or proposal submission and modifications required to integrate the final results and conclusions into a self-contained comprehensive and logically structured document.

Data Item Description
Status Report (1st to be submitted to the cognizant ONR S&T PO &/or Technical Point of Contact within 30 days after a ward (DAA), and 2nd on or about 90 DAA informal; typed or printed on 82 x 11 inch paper)
 
Front cover with title (Solicitation topic number), principal investigator=s name, firm name and address, contract number, specified Navy Small Business Innovation Research Program distribution statement, period covered by the report, serial number (see instruction for voucher, SF 298), security classification (e.g., UNCLASSIFIED), and name of the issuing government activity (e.g., Office of Naval Research or Naval Air Systems Command).
Informal description of significant progress made against specific milestones during the reporting period (positive and negative results with conclusions and recommendations, as required).
 
Identify any technical and/or programmic issues possibly affecting completion of the purchase order/contract; e.g., availability of residual funds and/or delayed tasks.
(Document current and overall person-hours and cost expenditures relative to plans and any significant changes to the contractor's organization, operating procedures, project management network, and/or to the milestone chart.)
 
Provide appendices for any necessary tables, references, photographs, illustrations, and/or charts: Records of significant trips, telephone calls, and/or transition commitments; Plans for key activities during the following reporting period; and Name, telephone number, FAX number, and e-mail address of preparer of the report.

Kingman Road, Suite 0944; Ft. Belvoir, VA 22060-6218), and 1 copy to the Naval Research Laboratory (ATTN: Code 2627, Washington, D. C. 20375-5326)-- readable fonts printed on 82 x 11 inch paper with 1-inch margins, as required)

Report Documentation Page Standard Form (SF) 298 may be down-loaded from ONR/Contracts and Grants/University Business Affairs Information/Forms website (http://www.onr.navy.mil/02/024/forms/). SF-298 Block 12a (Distribution/Availability Statement) in each unclassi-fied final report must contain one of the following statements:
 
A. Distribution authorized to U.S. Government Agencies Only; report contains proprietary data produced under SBIR contract. Other requests shall be referred to the performing organization in Block 7 of this form; or
 
B. Approved for public release; SBIR report, distribution unlimited.
The final report abstract (Block 13 of the SF 298) must identify the purpose of the work and briefly describe the nature of work carried out, the finding or results, and the potential government and private sector applications of the effort (since this abstract may be published by the DoD, it must not contain any proprietary or classified information or data).
 
Descriptions of scientific/technical objective, approach(es) taken , work completed (e.g., physical and analytical models developed and analyses conducted), positive and negative results (e.g., findings substantiated by tabulations, photographs, illustrations, charts, and/or references, as required to permit their independent verification), conclusions, and recommendations.

GUIDELINES FOR PREPARATION AND SUBMISSION OF ONR SBIR PHASE II PROPOSALS
The following guidelines were updated on 13 February 2003 for the Office of Naval Research Small Business Innovation Research (ONR SBIR), a program managed by the Industrial and Corporate Programs Department SBIR/STTR Division (ONR 364) in conjunction with Navy & Marine Corps Science & Technology (S&T) Programs as described on the ONR SBIR Home Page
(http://www.onr.navy.mil/sci_tech/industrial/sbir_onr.htm). While many SBIR awards are administered by ONR on behalf of other subcomponents of the Defense Department; e.g., the Missile Defense Agency (MDA) and the Defense Advanced Research Projects Agency (DARPA), these guidelines are provided for proposals to be supported in whole or in part under the ONR portion of the Navy SBIR Program (Element #0605502N).

SECTION 1 - ELIGIBILITY AND LIMITATIONS
Only those companies which have executed a Phase I contract and have been invited formally or informally by the Navy Science & Technology Program Officer (S&T P0), technical point of contact (TPOC) for the Phase I contract, or sponsoring organization Program Manager (PM), are eligible to submit a follow-on Phase II proposal. ONR reserves the right to make no award, one award, or more than one award in any topic as U.S. Navy & Marine Corps R&D requirements evolve.
 
Each proposing firm must:
 
・Continue to qualify as an independently, U.S. citizen (or lawfully admitted permanent resident alien) owned and operated small business qualified for research or research and development purposes, as defined in Section 2 of the most recent DOD Solicitation, and certify to this on the Cover Sheet (Appendix A) of the proposal.
 
・Perform a minimum of one-half of the research or research and development effort (Phase II);
 
・Be the primary employment of the principal investigator at the time of award and during the conduct of the proposed effort (primary employment means that more than one-half of the principal investigator's time is spent with the small business).
 
・Perform the research or research and development within the United States. "United Sates" means the fifty states, the Territories and possessions of the United States, the Commonwealth of Puerto Rico, the Commonwealth of the Northern Mariana Islands, the Trust Territory of the Pacific Islands, and the District of Columbia.
 
Joint ventures and limited partnerships are permitted, provided that the entity created qualifies as a small business in accordance with the Small Business Act, 15 USC 631 and the requirements of the most recent DOD Solicitation, and provided that the Phase I contract was awarded to the joint venture or partnership.
Note: Any deviation from these requirements must be explicitly approved in writing by the Contracting Officer during negotiations.
