Pleural TB: Two Types

　

・TB pleurisy

- Small number of organisms enters pleural space

- Development of hypersensitivity response: neutrophilic then lymphocytic

- Effusion usually small & unilateral; occasionally large

・TB empyema

- Large number of organisms enters pleural space from rupture of cavity or parenchymal focus

- Fluid thick and cloudy; high number of WBC

- Does not resolve spontaneously; residual scarring
