Registration:

^*1 Panelist, Chairperson/Co-chairperson of each panel discussion.

^*2 Banquet will be free for accompanying spouses, assigned participants, Chairperson/Co-chairperson of each session.

　

Registration Fee Covers:

Application:

Application should be made by submitting the enclosed registration Form A to the Registration Secretariat for reservation by the 30^th of September. Please complete the registration Form A fully and send to the Secretariat by fax or regular mail. All participants including invited speakers and chairpersons must submit the Form A. Only these registrants who have registered by 30^th of Sep., 2000 will be included in the list of participants. Registration will also be possible at the symposium site. However, since the registration desk is expected to be crowded, please make every effort to register in advance.

Please make separate application for each participant. For additional application Forms, please make copies of the original and use them.
