In the pathogenesis of non-insulin-dependent (type 2) diabetes at least four important factors can be discerned, that is: a) individual or ethnic factors causing proneness to diabetes development, b) impaired function of islet beta cells, c) resistance of tissues to insulin and d) environmental factors. The environmental factors, such as low physical activity, central type obesity, high-calorie diet with fat content, increase insulin resistance and contribute to diabetes manifestation in individuals with genetic predisposition.

Purpose: Special training program as primary prevention in persons with high risk of diabetes type 2.

Methods: epidemiological, year-long study of 110 individuals from population with high risk factors of diabetes type 2 who were subjected to a special training program (individual physical training relying on exercises of intensity 50% VO2max during 20 minutes, three times per day, seven days per week; assessment control of training program on diabetes treatment effectiveness by biochemistry, psychological and anthropometrical criteria.

Results: Six month of the individual training program caused: a) improved in lipid profile (biochemical investigations of blood), b) increased a physical efficiency (direct measurement of VO2max), C) increased quality of life (psychological special test), d) decreased fat body mass (anthropometric investigations), e) possibility of cease of oral administration hyperglycemic medications for 36% of patients.
