わが国でも老年医学の専門医は存在するが,老年医療を専門診療科として標榜する医療施設は少ないし,老年医療を専門とする訓練機関や施設もほとんどない実情からみて,高齢化したわが国における老年医療は著しく後れをとっていることは否めない。今回のボストン,およびニューヨークにおける老年医療を専門とする施設の見学を通じて,わが国においては高齢化した社会におけるより適切な医療のあり方が早急に検討されなければらないが,まずは問題の所在を明らかにし,その解決策が立てられ,そして,実践を通じてより質の高いレベルに発展させる戦略を展開する必要があると思われた。

　2050年には全人口の35%が65歳以上の高年者で占められることから,身体的,心理的,社会的に活動性の高い,より生産的な高年者の数が増すことは確実であるし,そのような人たちがより積極的に社会的役割が果たせるような社会の仕組みに変えなければならないが,更にfrailでvulnerableな高年者群には適切な全人的な機能評価のもとに身体的,心理的・精神的機能を高めながら,より活動的で生産的な社会的役割が維持できるように統括されたケアやサポートを供給し,そして,不幸にして終末期に至った最老年者には,適切な全人的機能評価のもとに必要となるケアを効率よく提供できる社会的資源利用の基準を定め,尊厳性が高く維持される終末医療を実現することが必須であると考える。そのような見地から見れば,今日行われている老人健診は一般成人の延長上にあるもので,老人のリスク層別化に何ら役立つものではない。真に社会的に必要な老人の評価を行うことによって,よく生きるための生活の選択における指針を示し,ともに考え,支援しながら人生の終末に到達できるような医療の実現が切に望まれる。

　今回の医療事情調査訪問においては,「Living Well」を著したDr.Jeanne Weiや,「Living to 100」の著者であり,長寿遺伝子の研究でも知られるDr.Thomas Perlsとも意見を交わし,また,BIDMCのChief of General MedicineであるDr.Thomas Delbanco,Dr. Russell Philipsからはプライマリ・ケアにおける教育のシステムと研究の成果について多くの情報を得たことを付記する。
