Achieving high cure rate and high case finding country wide can be obtained by applying DOTS Strategy. Some major points have to be emphasized:

-The government commitment at all level is important.

-The quality of the laboratory network at district level.

-The managerial treatment system.

-A regular drug supply of quality and free of charge for the patient.

-A built in system of supervision, monitoring and cvaluation with a training programme.

-DOT can be used not only for SCC but also for old standard regimen.

 

Sir John Crofton, the greatly respected pioneer of Antituberculosis Chemotherapy, has remarked that more than thirty years after the method for care and prevention were evolved and before the advent of the HIV epidemic, there were already more patients with active tuberculosis in the world than there had been in the 1950. The reason of this situation may be that the minority of patients receive a complete course of effective therapy. This means that the compliance or adherence to treatment is very important, then undubtedly DOTS would be the answer.

There is much more to this than watching the patient swellow some tables.
