-Hemostasis (Tourniquet use)-
This is a method effective against massive bleeding from an extremity in which the extremity is tightly bound at a site more proximal than the site of the bleeding (see the figure). At this time, follow these guidelines:
1]Do not use a thin string or elastic string for binding. A rubber Esmarch bandage, or the like is preferable.
2]Bind the upper arm or thigh (the forearm and the lower leg cannot be bound tightly because there are two parallel bones between which an artery is located).
3]Loose binding results in no hemostasis in the deep artery and binding of the superficial vein alone, which may lead to a larger amount of hemorrhaging.
4]A long period of binding carries the risk of neuroparalysis or muscular necrosis that produces sequelae. If the binding is continued for more than 2 hours, relieve the binding temporarily every 30 minutes.
