THE EFFECTIVENESS OF DICOFENAC SODIUM IONTOPHORESIS IN CASES OF KNEE OSTEOARTHR ITIS WITH ACUTE INFLAMMATORY REACTION AND THE COMPARISON OF THE RESULTS OBTAINED IN PLACEBO AND THERAPY GROUPS

A group of 20 patients consisted of 2 males and 18 females with a diagnosis of knee osteparthritis confirmed by clinical and radioloigic findings were randomly divided into two groups of 10 patients. Following a washout period of 15 days for the drugs which have been taken previously, the first group was treated by diclofenac sodium (Voltaren amp. 75mg) lontophoresis 5 days a week for a total period of 10 days. Synovial fluid and venous blood samples were collected immediately after the first application to be analyzed at EMRI (Experiment Medical Research Institute of Istanbul University) by HPLC (high performance liquid chromatography. The second group received placedo iontophoresis with disitilled water in same manner. For ethic reasons no blood or synovial fluid was collected in the placedo group. The clinical criteria used for the evaluation of the results consisted of the VAS determination of morning stiffness and the time for walking a distance of 10 meters.

The results may be summarized as below. No diclofenac sodium could be traced in the venosus blood samples, but it could be traced in the synovial fluid of all of the patients in a range 3.32-129.06ng/ml. (Peak area). During the initial evaluation the clinical criteria did not show a statistically significant difference (p 0.05) between two group. At the and of the therapy programme an evident statistical difference (p 0.001) was found between two groups in view of the VAS determination and the walking speed. Algometric measurements, knee range of motion and the duration of morning stiffness also showed statistically significant difference (p 0.05) between the placebo and therapy groups.

In conclusion it can be said that the iontophoresis treatment may be considered as an effective non-invasive method for penetration of diclofenac sodium ions into the tissued without using the way of ------tion.

STUDYING THE EFFECT OF ULTRAPHONOPHORESIS WITH "OSTOCHONT"-SALBE PREPARATION AT SOME LOCOMOTION SYSTEM DISEASES

The study aims to establish the effect from application of "Octochont"-salbe preparation (Adenylchemie GmbH -1000 Berlin 10) by independently applied ultraphonophoresis on 30 patients with epicondylitis, tendovaginitis, gonarthrosis.

The effect of the therapy applied has been traced by objective and subjective indexes and the following tests have been used: pain intensity, initial stiffness, palpation, inflammatory process dynamics, anglemerry, santymetry of active and passive toovents in joints etc. All data have been statistically processed by methods of alternative and variation analysis.

The new method suggested for treatment with "Ostochont"-salbe ultraphonophoresis can be used successfully for reducing pain and synovitis followed by improvement of functional capacity.
