FEATURES OF FLOOR REACTION FORCE IN GAIT OF PATIENTS WITH PARAPARESIS AND ITS RELATION TO SPASTICITY

The purpose of this study was to clarify features of floor reaction force in gait of patients with spastic paraparesis and to determine its relation to degree of spasticity.
25 patients with spastic paraparesis caused by cervical cord compression were studied. To determine the relation to the degree of spasticity, the subjects were classified into 3 groups assessed by the degree of clonus (severe: 9 cases; moderate: 8 cases; mild: 8 cases). All patients were made to walk barefoot at cadence 80 on a force plate set in the center of the walking floor. The productive reaction force in the force plate in the stance phase of the subjects' right and left limb was divided into three-dimensional components (Fx, Fy and Fz). These were amplified and recorded on a pen recorder as time course data. Among the recorded floor reaction force waves, the wave forms of [X], [Y] and [Z] were analyzed.
In all patients who had severe clonus, 2 peaks of the fore component and snaked change of the curve were seen in [Y]. The degree of clonus were correlated statistically with 6 items, that is snaked change of the curve in [X], initial spike, 2 peaks of the fore component, snaked change of the curve, severe deformation in [Y] and isolated small peak at the early stage of stance phase in [Z]. It was appeared 5 items of 6 in severe clonus cases, 3 items in moderate cases and 1 item in mild cases on average.
Several specific wave patterns of floor reaction force in the gait of patients with spastic paraparesis are related to spasticity directly or indirectly, and the state of their appearance correlates with the degree of spasticity.

The improvement of gait parameters in patients after total hip arthroplasty

The purpose of this study is to examine the improvements of postoperative gait. The gait parameters in six female patients with unilateral osteoarthritic hip joint were analyzed before, and 11-15 months after, a total hip arthroplasty by the three-dimensional gait analysis system using multiple landmarks. The kinematic and kinetic data were calculated. A part of the results are summarized in Table 1. Joint angular displacement of hip flexion- extension were improved significantly after operation (p<0.001) and those increased step length. Cadence was not increased, and so the factor for improvement of gait speed was the increase of step length. Displacement width of center of gravity (COG) showed decrease. We consider that gait analysis is one of the important and objective evaluation for the effects of operation and exercise.
