THE GRITTY'S AMPUTATION AS A METHOD OF CHOICE FOR TREATMENT OF PATIENTS WITH CRITICAL LOWER-EXTREMITY ISCHEMIA

In our study we aimed at lowering the amputation level in patients with severe ischemia of the lower-extremity vessels and restoration of the walking stereotype when wearing a prosthetic appliance by shaping thigh stump where adequate circulation was repaired.
Thirty eight patients with femoral arteries occlusion and severe lower-extremity ischemia were involved in our study. Endarterectomy from deep artery of the thigh and profundoplasty were performed to repair circulation. Original devices made of NiTi alloy with shape-memory effect was used for compressive patella and femur fusion in the Gritty's amputation.
Combination of the repaired circulation in the deep artery of thigh and shaping of the long thigh stump allows to perform prosthetic management by what the distal end of the stump is bearing.
In adequate axial load on the segment operated there appear conditions for development of collateral circulation in the stump and repairment of the manner of walking with the prosthesis.

A 7-year-old Patient with Hip Disarticulation

PURPOSE: We evaluated the process of overcoming the severe physical and mental trauma in a 7-year-old child.
METHODS: We interviewed the medical staff and the family of the patient, and reviewed the chart.
RESULTS: The coordination between the medical staff as to the mental states of the patient, which changes everyday is very important. Seeing the passion of the medical staff, the patient and the family made effort to overcome the disorder. The school made it possible for the patient to attend.
CONCLUSION: The success of the walking with a prothesis and the recovery of ADL is not sufficient for the patient to accept the disorder. Our findings revealed the importance of coordination sufficient between the medical staff concerning mental state of the patient.
