Role of Orthotic Treatment in Acute CVA Rehabilitaion

Purposes of orthotic application for lower limb for stroke patients are to control spasticity, to prevent contracture and to make them walk better. Plastic AFO (ankle foot orthoses) are recently more utilized than conventional metal AFOS. They were tend to be prescribed in subacute or chronic stage from the onset of stroke.

In our institution, active orthotic treatment were tried in thc acute stage and found out that the conventional AFO is more effective to provide more stability and to facilitate motor recovery and better ambulation than plastic AFOs. Their function can be modified easily in clinical basis with fixed or plantar flexion limit in double Klenzak joint.

Conventional KAFO (knee ankle foot orthoses) were applied to patients with complete flaccid paralysis and/or trunkal instability which are frequently accompanied with unilateral spatial neglect. With recovery of function they were cut down to AFO. It is suggested that early orthotic application is the key for acute stroke care.

DYSPHAGIA REHABILITATION IN JAPAN

Dysphagia is a serious problem for many of the disabled, but its rehabilitation is relatively a new one. In this paper, I will give an overview of dysphagia rehabilitation in Japan.

During the period from 1980 to 1985, a rehabilitation approach to dysphagic patients began in Japan, but it was not common even in advanced rehabilitation hospitals.

In 1986, a videofluorographic evaluation was introduced to this field. Since then, many studies about dysphagia have been presented.

By about 1990, a systematic rehabilitation approach began in some hospitals, and some new techniques including an intermittent catheterization were developed.

The establishment of the Japanese Society of Dysphagia Rehabilitation in 1995 encouraged an interdisciplinary approach to disseminate.

I will also refer to some other topics relating to dysphagia rehabilitation in Japan.
