SHOULDER SUBLUXATION IN HEMIPLEGIA: COMPARISON OF 4 SLINGS

Purpose: To compare the effectiveness of the Rolyan, hemisling, remodified Bobath, and the newly designed triangular Bobath sling.

Method: 16 patients with shoulder subluxation had taken simple shoulder AP X-rays without and with 4 slings, respectively. The hemisling was applied with elbow flexion at 90 and 120 degrees. The vertical and horizontal displacement were measured by X-ray.

Result: The vertical displacement was corrected significantly by triangular Bobath sling and hemisling at 120。?elbow flexion. The triangular Bobath sling did not correct the horizontal displacement but improved the discomfort of the axilla compared with the remodified Bobath sling.

Conclusion: Among 4 slings the triangular Bobath sling was the best in this study, but more concern about reducing lateral displacement and improving applicability is necessary.

MEDICAL COMPLICATIONS DURING STROKE REHABILITATION

The value of rehabilitation for post-acute stroke patients is now widely accepted. The outlook for most patients is generally favorable. However medical complications are frequent, and these can negatively impact the participation of the patient in the program. The consequences may be less favorable outcome or shortened survival. It has been estimated that the frequency of medical complications is up to 60% for all stroke patients, and as high as 94% for patients with severe lesions.

The optimum rehabilitation of a patient with a recent stroke must therefore include an appropriate level of medical and nursing monitoring to ensure that any coexisting disease is appropriately treated, and complications are prevented or identified early and treated promptly.

This paper will discuss in detail the management of medical complications occurring during stroke rehabilitation and will provide an overview of management of co-existing diseases. The issue of secondary stroke prevention will be discussed.
