ORTHOTICS, RECENT DEVELOPMENTS, PRESCRITION AND PRACTICE

Ellis Margaret (West Square Associates, London, SE11 4SP, UK.)

An Orthosis is "a support surrounding a weakened bodily segment to support or replace a missing function". Pain, contracture and deformity can be avoided by us of orthoses. Mobilization can take place at an earlier stage through orthotic usage where fractures, other traumatic injuries and surgical procedures have taken place. Acute conditions such as infections, burns and oedema as well as more chronic conditions such as morning stiffness in rheumatoid arthritis, neurological problems in spinal injuries and stroke, lack of sensation and range of motion can be also be assisted. New materials enable a wide range of static and dynamic orthoses some of which will be illustrated.

This presentation will review work being undertaken and also comment on the value of education by team members. The role of ISPO in orthotic education and training will also be emphasised.

Lower limb orthotic management for hemiplegia

Hideo Watanabe (Saga Medical School, Saga, Japan)

We have been prescribed plastic ankle foot orthosis (AFO) for the majority of the CVA hemiplegia patient. The purpose of prescription of the plastic AFO in early recovery stage is to prevent ankle and foot deformities and to encourage ambulation. The conventional type AFO with two-direction control adjustable ankle joint is prescribe to the patient with weak knee extension in ambulation. For the patient with very unstable knee in standing, we use knee ankle foot orthosis with ring lock knee joint. We have several kinds of assembling orthosis in the rehabilitation division, and the patient can stand and walk in parallel bars with the orthosis at early stage of CVA rehabilitation.
