STUDY ON THE RELATION BETWEEN SELF ASSESSMENT OF DAILY LIFE ACTIVITIES AND INSTRUMENTAL ACTIVITY OF DAILY LIVING OF THREE YEARS LATER

There were several studies on the relation between psycho-social factors and instrumental activities of daily living (IADL) for the elderly in communities. The purpose of this study was to clarify the relation between self assessment of daily life activities and IADL of three years later. We started comprehensive longitudinal study in 1991. Nine hundred fifty-two persons of 60 years old or above living in farming communities were interviewed about psycho-social factors such as communication with community members, situation of their health, life style and self esteem, and TMIG Index of Competence (IADL).

The results were as follows. 1) Self assessment of daily life activities was classified into 5 factors by factor analysis of 1991 data. i.e.: a) Independence (Active approach, life style motivation, health motivation regular life style), b) Social curiosity (Using electronic equipment, having a hobby, reading books, reading newspapers, feeling of importance), c) Interaction (Communication with outsiders, interaction with outsiders, communication within the family), d) Participation in the society (Participation in social groups, participation in neighborhood affairs, Watching television, Having active role), e) Feeling of safety (having counsel, having someone to give support in emergency). The cumulative proportion of those 5 factors was 53.6 and Cronbach α was 0.78. 2) Self assessment of daily life activities was highly correlated to IADL score of the same year. 3) Low level of social curiosity, and poor interaction were related strongly to decrease of IADL score of 3 years later. 4) Among 18 items of self assessment of daily life activities, "active approach" and "feeling of importance" were related strongly to decrease of IADL score of three years later.

In conclusion, self assessment of daily life activities was a reliable methods to predict IADL of three years later for the elderly in communities.

A CONVENIENT METHOD FOR MEASURING DAILY PHYSICAL ACTIVITIES

The purpose of this study is to presume the energy consumption of daily physical activities in the daytime by using a three-dimension accelerometer. Physical activity, being an important part of human behavior, may be related to various aspects of health and diseases. In this study, the relationship between the intensity of each action and the three-dimension accelerometer output was compared in 10 healthy subjects under the free living conditions. Subjects wore to the three-dimension accelerometer on their nondominant wrist during daytime and filled out a physical activity check list (self-report). Oxygen consumption was measured for each subject while walking on the treadmill at the comfortable walking speed. The three-dimension accelerometer output appeared to correlate highly with the intensity of actions by the Spearman Rank test. The average of the oxygen consumption was 14.0+/-2.4ml/kg/min and the average of the accelerometer output was 260/min at the comfortable walking speed; 4.9+/-0.7km/h. The increase of energy consumption by physical activities in the daytime was able to be presumed by the following calculation; energy consumption (kcal)=amounts of the accelerometer output/260 * 14.0(ml/kg/min).* body weight(kg) * 60(min)/1000(ml) * 4.825(kcal/l). This method was very convenient and useful when advising patients about the exercise prescription.
