NERVE CONDUCTION STUDIES AFTER ENDOSCOPIC CARPAL TUNNEL RELEASE USING TWO PORTAL TECHNIQUE

Sensory and motor nerve conduction studies were performed in 16 wrists of 14 consecutive patients with idiopathic carpal tunnel syndrome before and after the surgery of Chow's two portal technique to see if the procedure effectively decompressed the median nerve.

Distal motor latency (DML) and sensory conduction velocity (SCV) were recorded before and at 1, 3 and 6 months after the surgery. We followed Chow's technique except positioning of the entry portal. We cut a few millimetres of the proximal part of the transverse carpal ligament longitudinally under direct vision through a proximal incision over the wrist crease. This allowed the slotted cannula to go through the tunnel with minimal resistance and eventually would prevent the median nerve and the tendons from intervening between the ligament and the slot. DML could be detected in 12 wrists and SCV in 9 wrists before the surgery. The averages of DML were 7.3ms±2.0 before the surgery, 5.6±0.96 at 1 mon., 4.8±0.74 at 3 mons, and 4.6±0.71 at 6 mons after the surgery. Those of SCV were 30.8m/s±6.9, 37.5±4.5, 41.1±2.2, and 43.7±2.3, respectively. It was shown that both DML and SCV improved, especially in the first month postoperatively (p<0.05). This finding was consistent with that of the conventional open procedure which had been shown in the literatures. It was objectively demonstrated that the Chow's two portal technique was effective to relieve the median nerve from compression.

NEW INDEX AND ANGLES TO DESCRIBE OF LORDOTIC CURVES.

This research explains the effectiveness of some new parameters on radiography pictures with high validity by both statistical analysing and mathematical calculation for evaluating of lordotic curvatures of spinal column. Radiography pictures of 31 volunteer subjects were taken in normal standing position, then the proposed and two conventional parameters in measuring lumber curve were estimated on radiography pictures, and compared to each other in order to evaluate the validity of the suggested parameters.

The statistical and mathematical results represent that with P<0.001, the suggested parameters in this study such as χ.E.α'.α.β.γ angles and N index and their mathematical relation with correlation from r=0.80 to r=0.95, are valid for the evaluation of lordotic curvatures.
