TRIAL OF THE INSTRUCTIONS FOR ACTIVITIES OF DAILY LIVING (ADL) AGAINST THE JOINT DEFORMITY FOR RHEUMATOID ARTHRITIS (RA) PATIENTS.

In order to investigate the protective effects of the instructions for the activities of daily living (ADL) against the joint deformity for the rheumatoid arthritis (RA) patients, 34 RA patients were reviewed average 22.4 months after the initial instructions. Instructed ADLs are composed of 18 different kinds of activities such as the safety way to stand.
In terms of the acceptance, 78% of total patients were satisfied with these instructions and 60% have expected to prevent the progress of the joint deformity due to RA after 6 month follow-up periods. Though these instructions did not affect the disease activities of RA, one third of the non-destroyed wrist joints (Steinbrocker's stage I) were suffering from the erosions and half of the un-deformed joints (Steinbrocker's stage II) yield to joint subluxation unfortunately. Consequently, they could not always inhibit the progress of the Steinbrocker's stage of RA patients rentogenographically after 12 months. However, the extent of the articular destruction of their wrist joints of early RA patients were relatively slower than those of late stage of RA patients.
These results suggest that when the instructions of the ADL for the RA patients were applied for RA patients as soon as possible after their disease onset, they might be effective for the delay of the joint destruction due to RA though they could not stop the progress of the joint deformity completely.

EFFICIENCY OF COMPLEX REHABILITATION IN OLD PATIENTS RHEUMATIC FOOT

The complex rheumatic foot at the old patient is an important degenerative pathology, with implication in the lower limb function.
Purpose: The study intended to prove the importance of physical rehabilitation in the algofunctional syndromes of the old patient rheumatic foot.
Method: We studied 36 old patients with complex rheumatic disorders of foot. The clinical syndromes were dominated by the pain, limitation of mobility, difficulties of statics and walking. We proposed a personal score of valuation and we applied an unitary programme of thermo-electro-masso-kinetic therapy, during 20 days, following the clinical evolution. Later, we applied also sole supports for maintaining the results.
Results: We obtained an improvement between 50-75% of clinical parameters (pain, mobility, statics and walking) and a long remanence of the results. We propose a practical clinical score and a unitary programme of rehabilitation for these patients.
Conclusions: An unitary programme in the complex rehabilitation of old patients rheumatic foot is necessary for maintaining the global function of lower limb and the individual autonomy.
