ORTHOSIS THERAPY FOR HALLUX VALGUS

Abstract: Arch supports were used for the treatment of hallux valgus in 154 patients (145 female, 9 male) between 1989 and 1995.

Indication of this orthosis therapy is decided by the position of sesamoids of the first metatarsophalangeal joint. The indicated case has no dislocation of sesamoids.

After this therapy, the subjective assessment of every patient was graded as satisfying.

X ray investigation showed that no shifting of the sesamoids and the normalizing of hallux valgus angle at the first metatarsophalangeal joint.

EFFECTS OF ORTHOTIC INSOLE ON GAIT IN PATIENTS WITH HINDFOOT TRAUMA

Trauma to the foot might cause various degree of damage to soft tissue and/or bone. With the advancement of surgery, limb salvage could be available by proper bony fixation and reconstruction of soft tissue. However, the reconstructed soft tissue was usually not strong enough for weight bearing during ambulation, sometimes, pressure point was created at the lesion site, resulting in skin ulcer, foot pin and walking difficulty. Fourteen cases with hindfoot trauma were recruited. All of them had received soft tissue reconstruction surgery. Total contact insole (TCI) with heel elevation was given to redistribute body weight in order to promote ulcer heeling, decrease pain and improve gait. Three dimensional gait analysis and in-shoe foot pressure were assessed before and after wearing the TCI. Marked increased in mechanical energy generation during stance phase and improvement in gait symmetry were noted.
