REHABILITATION IN ACUTE STROKE: AN INTERNATIONAL COMPARISON

Rehabilitation after acute stroke differs in various countries. For Germany and other European countries the post-stroke interval until admission in a rehabilitation center, the rehabilitation facilities, the applied assessment tools, the length of stay, the methods of physical therapy, occupational therapy, of neuropsychological training and speech therapy, the discharge planning, the outpatient rehabilitation, the prescription of technical aids, auditing of rehabilitation hospitals, quality control and other detail of poststroke rehabilition are discussed in an international comparison. Recent reforms of the health system in many countries resulted in a major shift in acute stroke rehabilitation which also will be discussed.

Stroke Rehabilitation, changing Values and Outcomes

In the 1970's we were convinced that restoration of survival skills was the most important rehabilitation goal in Stroke Illness. During the last two decades we have become increasingly aware that in practice, restoration of role and lifestyle is a more important goal and a highly valued outcome. We have now confirmed that prolonged invalidity is, paradoxically a common sequelae of intensive physical rehabilitation following stroke. We now have evidence that this complication of rehabilitation is preventable. With the increased prevalence of stroke disability in many countries it is perhaps the most important socioeconomic cost of poorly planned and executed rehabilitation programs.
