APPENDIX 6: WORKING GROUP INSTRUCTIONS
Instructions for Formations of Working Groups
Aim:
The groups are being formed to → increase participation
→ develop plans of action with emphasis on the areas specified
→ develop specific recommendations which should include strategies and assignment of responsibilities
→ encourage exchange of experience between countries
To accomplish the above aim, the participants will be divided into groups, by country, and each group will cover the following areas:
1. Performance of RDFs, including guidelines and procedures for RDFs, inventory management, accounting and financial management, etc.
2. Monitoring and supervision, including strengthening the management capacity of health teams at various levels to plan, implement, coordinate activities and train and provide health education on proper use of drugs
3. Community control of locally generated revenues resulting from health services and RDFs, including national and local policy and procedures regarding use of local revenues and the principles and methods for community oversight by PHC/RDF committees
4. Scaling up from pilots and first phases, exploring constraints, opportunities and time frames
5. Integration of primary health care services at various levels as a consequence of the RDF program
Guidelines for Working Groups Discussions
The specific objectives of the working groups are:
I. Review current progress pertaining to the RDF project in your country based on previous reports, the following list and your presentation, and identify issues, challenges and constraints to be addressed during the next implementation period.
 
List the specific objectives for the current reporting
These are to be derived from the plan of action and activities that were to be carried out during the reporting period.
Achievements against each objective for the current reporting period
Briefly describe the achievement against each objective (or activity) for the current reporting period and also state how this achievement contributes to the accomplishment of the overall objectives of the project or other projects that have a direct bearing on the RDF project.
General status of the RDF project using at least the following indicators:
number of RDFs established
number of RDFs functioning
number of RDFs not functioning, listing the reasons why
 
(in RDF operations)
whether or not drugs are being sold or given free whether the funds are being revolved or not whether there is an increase or reduction in the capital and stock the extent to which the community is involved in the operation and oversight of the RDF the extent to which the replenishment system is working in terms of
requisitioning, purchase,
 receiving, inventory management and storage of drugs
the extent of supervision provided by MOH, NGOs, UNICEF
what financial reports on RDF operations are being kept; are the following included?
starting stock
monthly sales
monthly drug purchases
other expenses during the month for the operation of the RDF
cash balance at the end of the month
cash in bank at the end of the month
stock value at the end of the month
total value of the RDF at the end of the month
increase or decrease of total value over the total value of the previous month
value of any loss, pilferage, damage or credit provided but not collected
  how these financial reports are being used by the community, health committees, MOH,
NGOs, UNICEF
  how the drugs are being priced at the local and higher levels
who the prescribers are and what they can prescribe
what other reports about the operation of the RDFs are being sent to MOH, NGOs, UNICEF
 
II. Categorize, under the five categories listed below, the identified issues, challenges and constraints.

Community control of locally generated revenues resulting from health services and RDFs
Scaling up from pilots and first phases, exploring constraints, opportunities and time frames Integration of primary health care services at various levels as a consequence of the RDF program
 
III. Develop a list of actions that would need to be taken over the next implementation period using the issues, challenges and constraints identified, employing the questions provided.
 
Performance of RDFs
 
○What sort of areas should be covered by the guidelines for improving the operations of RDFs?
○What sort of procedures and forms could be developed that would improve the performance and self-assessment of the RDFs?
○What should be the minimum elements of inventory management that should be required for effective performance of the RDFs at the community, district and provincial levels?
○What should be the minimum requirements for effective accounting and financial management of the RDFs?
○What should be the minimum requirements for setting up and implementing the government exemption policies for the operation of the RDFs?
○What should be the minimum components for routine self-assessment of the RDF operations?
 
Monitoring and supervision
 
○What sort of and in what areas should supervision be provided to monitor the operation of the RDFs?
○What should be the frequency for RDF supervision and who should be responsible?
○How can supervision of the RDFs be integrated into routine/regular supervision being provided by health management teams?
○How can the management and supervisory capacity of the health management teams be strengthened to support the operation of the RDFs?
○How should health education on the proper use of drugs be provided to the community and who should provide this education?
 
Community control of locally generated revenues resulting from health services and RDFs
 
○What should be the extent of community oversight over the operation of the RDFs and other revenue generating activities in the health sector?
○What principles and aspects should be included in the policies and guidelines that would regulate the use of these revenues?
○How should these policies and guidelines (to grant communities control over the management and use of locally generated revenues from operations of RDFs and health facilities) be developed, approved and implemented?
○What methods should be employed by community RDF committees to oversee the operation of the RDFs and other revenue generating activities?
○How should these community organizations be trained to use the guidelines and procedures to manage these locally generated revenues?
○How would these community organizations such as the RDF committees be monitored?
 
Scaling up from pilots and first phases, exploring constraints and opportunities
 
○What should be the minimum requirements to justify scaling up from a pilot project?
○What preparation should be undertaken to scale up from pilots or first phases?
○What type of issues and constraints should be taken into consideration when planning to scale up?
○How should the scaling up be planned and implemented?
○What sort of assistance will be required to scale up from pilots and first phases?
○What advice would you give to those countries that are in a pilot or first phase?
 
Integration of primary health care services at various levels as a consequence of the RDF program
 
○What effect has the RDF had on the integration of health services at the community level?
○How has setting up RDFs influenced or affected the management of district and provincial health services?
○What actions might need to be taken to encourage integration of health care service delivery at the community and district levels?
○What management skills would need to be developed to strengthen the health teams to support RDFs and to integrate primary health care services?
○What sort of policy changes would need to be made to incorporate RDFs into mainstream primary health care activities?
 
IV. Develop a plan of action for the next implementation period based on the actions listed, with time frames, resources required, cost involved, persons responsible and indicators to measure progress.
 
V. Formulate recommendations referring to the above areas considering your action plans according to the suggested format
 
VI. Present the action plan you have developed, along with the relevant recommendations at the plenary session.
Operation of Working Groups
The following steps are proposed. The working groups should carry out these steps to streamline their discussion to take advantage of the limited time available.
 
I. Select a moderator for the group who will moderate the discussion and group.
II. Select a rapporteur who will record the salient points of the discussion, prepare the presentation and draft and record recommendations using the format provided.
III. Select a spokesperson if the group decides that this task be done by a different person from the moderator or rapporteur.
IV. Review the role of the facilitator or resource person assigned to the group.
V. Review the areas and the questions provided.
VI. List the issues, challenges and constraints in each of the areas and identify required actions.
VII. Develop an integrated plan of action for the next implementation period based on the actions developed with time frames, resources required, costs involved (approximate), persons responsible and indicators to measure progress using the format provided.
VII. Prepare a presentation for the plenary session using the suggested format.
IX. Submit the action plan and the recommendations to the facilitator for inclusion in the conference proceedings.
 
Role of the moderator
 
1. Facilitate discussions
2. Keep discussion on the topic and area
3. Ensure that questions provided are utilized
4. Summarize and help the rapporteur formulate the action plan and recommendations
5. Encourage participation of all members drawing out the silent ones
6. Maintain a warm and congenial atmosphere
7. Resolve, with the assistance of the facilitator, any conflicts that may arise
8. Work closely with the facilitator or resource person, drawing in the resource person but ensuring that all participants have the opportunity to participate
9. Assist the rapporteur and spokesperson in preparing the presentation
10. Ensure that the objectives of the working group are accomplished
 
Role of the rapporteur
 
1. Record the salient points during the discussion
2. Record the prioritized list of issues, constraints and problems
3. Record all recommendations using the format provided for submission to the facilitator and presentation to the plenary
4. Prepare the presentation to the plenary of the group's report and recommendations using the format provided
5. Hand over the presentation, recommendations and notes of the working group to the facilitator after the presentation to the plenary
 
Role of the facilitator
 
1. Support and assist the moderator in his role
2. Act as a resource person providing the information and data when required by the group
3. In collaboration with the moderator and rapporteur, ensure that the guidelines and formats provided are adhered to
4. Ensure that the environment for the working group is conducive to conducting a discussion
5. Liaise with the coordinating facilitator (the conference moderator) as and when necessary, to ensure smooth functioning of the working group
 
Format for the presentation to the plenary
 
1. Brief summary of current situation of the RDF project
2. List of issues, challenges and constraints categorized by area with a brief explanation why these were selected
3. A plan of action for the next implementation period based on the actions developed, with time frames, resources required, persons responsible, indicators to measure progress and cost involved
4. List of recommendations and rationale using the format provided for the recommendations
5. Brief conclusion about working group performance and impressions
 
Format for drafting recommendations
 
1. State the actual recommendation. Do this by asking the question: What are we recommending?
2. The strategies that may be employed. do this by asking the question: How do we, broadly speaking, implement this recommendation?
3. The person/agency/institution who will be responsible for ensuring that the recommendation is implemented by asking the question: Who will be responsible for carrying out the recommendation or making sure that the recommendation is carried out?
4. Propose an appropriate time frame for implementation of the recommendation by asking the question: Approximately, by when should this recommendation be carried out?
